Why are pg number references incorrect?

Why do my heading references and other results of the various GML reference tags sometimes generate the wrong page number information?

Answer

  Try running with three formatting passes. On the first pass, DCF makes a
  guess at how long the reference will be. If the guess is wrong, on the
  second pass the heading (or other references) may move to another page.
  Three passes will fix most problems, some require four.

  If four passes doesn't fix it, format with FPASSES(3) and SYSVAR (W
  filename). This will write references out to a file after the last pass.
  Print the document then edit the output reference file to reflect the
  proper page numbers. Script again with SYSVAR (R filename) with a single
  pass. The tweaked reference file (containing the correct page numbers for
  the references) will be used for the final run.
