Description

  This patch updates all of the Apex on-line and printable documents.
  This patch includes fixes for defects:

      Missing docs on "Migration from CMVC to CC"
      Added migration section to the getting started guide
      and added entries to index.

      The 'Control > Clearcase > Migrate' menu quoted in the doc
      doesn't exist

      Cross manuals not present in the Help>Print Manuals menu
