The quickest and easiest way to get and return software license keys
is to use AccountLink, Rational's on-line license management tool.

Only use this form if you DO NOT have internet access.
If you have internet access, use AccountLink for getting and returning license
keys. AccountLink makes getting license keys for remote or isolated machines
easier than ever!

To request or return a license by fax, complete this form in your web browser
and print it out. Then fax it to Rational Licensing Support at the numbers at the
bottom of this form.

Product and Host Information

Step 1 - Select Transaction Type Select whether you want to get a license key in order to use a Rational product or return a license key back to your Rational account.	I would like to get a license key I would like to return a license key back to my Rational account
Step 2 - Locate your License Key Certificate	Your License Key Certificate was included in your software shipment. It contains information needed in order to complete this request.
Step 3 - Enter your Rational Account Number Your Rational account number is found on your License Key Certificate	Enter Your Rational Account Number
Step 4 - Select License Type The license type of your product is found on your License Key Certificate.
Step 5 - Select your Rational Product The product you purchased is found on your License Key Certificate. If you purchased multiple products you can use the table at the bottom of this form to register multiple products to a single machine.
Step 6 - Enter Quantity Enter the quantity of licenses you wish to add or return. If you're adding licenses to this host, the quantity should not be greater than the number of licenses you have purchased. The quantity purchased is found on your License Key Certificate. If you are returning licenses back to your Rational account, the quantity should not be greater than the quantity which you already have installed. If you selected "Node-Locked" or "Node-Locked UNIX" as the License Type in Step 4, enter "1".	Enter the number of licenses to add to or remove from this host:
Step 7 - Select the Platform of Your Machine Select whether your license server or node-locked host is a Windows or UNIX machine.	My host is a Windows machine My host is a UNIX machine
Step 8 - Enter Host Information Rational needs the name of your host (your computer) and its hostid in order to process your license. To get your host information - On Windows, run the hostinfo.exe tool found in the <rational>\common directory. On Unix, run the command rs_hostinfo. Copy and paste the hostname and hostid into the fields. If you selected "Floating" or "Named User" as the License Type in Step 4, enter the host information of the license server machine. If you selected "NodeLocked" or "NodeLockedUNIX" as the License Type in Step 4, enter the information of the machine on which the Rational product will be used.	Hostname = Host ID = Use Ethernet Address (Optional for Windows Machines) Select this box if this host is a Windows machine and you wish to use the ethernet address as the HostID instead of the disk serial number. To get your host information - On Windows, run the hostinfo.exe tool found in the <rational>\common directory. On Unix, run the command rs_hostinfo. Copy and paste the hostname and hostid into the fields.
Step 9 - Redundant License Servers (OPTIONAL) If you have floating or named-user floating licenses and wish to use FLEXlm redundant servers, enter the host information for the secondary and backup license servers.
