Cúram Integrated Case Management focuses on assessing needs and delivering programs and services to meet these needs; Outcome Management assesses needs and designs plans to achieve sustainable outcomes.
Although this guide makes reference to Outcome Management, this guide primarily focuses on the needs to delivery process. The needs to delivery process is made up of two main parts: meeting needs through the delivery of programs in the form of benefits and meeting needs through service deliveries and referrals.
