Needs to Outcome describes a process of identifying the needs of a citizen/family, supporting them with relevant services and programs through to maintaining positive outcomes.
The following areas of the Application support this process:
- Triage - based on circumstances of the client/family, a
triage can determine community services and government programs that
citizens can access to help with their situation
- Screening - determines potential eligibility for social
programs/benefits
- Intake - either based on a screening/triage result or via
direct access, this is the process of applying for programs/benefits
(application)
- The application is reviewed, evidence verified and
confirmed
- Programs/benefits being applied for are authorised (or
denied if no eligibility)
- Ongoing Case Management - once programs and benefits have
been authorised, the ongoing interactions the clients have with the
organisation are managed within a case. Programs/benefits are created
following authorisation of the application and financial payments are
generated
- Outcome Management - based on the specific circumstances
of the family/citizen, a plan is created to assess the various factors
that need to be addressed in order to meet objectives such as
obtaining full time employment and an overall goal, such as becoming
self-sufficient
- Citizen Account - an account can be created for a client
so they can view interactions with the organization such as
activities/actions/messages relating to their outcome plan, services
and programs
